Claiming your EBB / Lifeline Credits

The information provided below will be used to verify eligibility for the Emergency Broadband Benefit (EBB) program and/or Lifeline Program. In order to claim credits, your application must be approved by USAC/National Verifier. Please wait until you received a response of “QUALIFIED” before starting the claim process below.
  • Ensure you have received an approval status of “QUALIFIED” from the https://getemergencybroadband.org (USAC National Verifier) site after you entered your information to verify eligibility.
  • Fill out the Kinetic by Windstream EBB/Lifeline Information form below with the information that was submitted and approved by https://getemergencybroadband.org (USAC National Verifier). This information must match exactly to prevent delays in having proper credits applied to your Kinetic by Windstream Account.
    Pro Tip! Take a screenshot of the information you submitted to https://getemergencybroadband.org (USAC National Verifier) and use that same copy to fill out this form to ensure an exact match!
  • Submit your Qualified Application ID and matching information via the Kinetic by Windstream EBB/Lifeline Information form.
  • Once your submission is processed and credits applied, you will receive an email confirmation from Kinetic by Windstream.
  • Credits to your Kinetic by Windstream account will appear on your next bill.
