Ensure you have received an approval status of “QUALIFIED” from the https://getemergencybroadband.org (USAC National Verifier) site after you entered your information to verify eligibility.
Fill out the Kinetic by Windstream EBB/Lifeline Information form below with the information that was submitted and approved by https://getemergencybroadband.org (USAC National Verifier). This information must match exactly to prevent delays in having proper credits applied to your Kinetic by Windstream Account.
Pro Tip! Take a screenshot of the information you submitted to https://getemergencybroadband.org (USAC National Verifier) and use that same copy to fill out this form to ensure an exact match!
Submit your Qualified Application ID and matching information via the Kinetic by Windstream EBB/Lifeline Information form.
Once your submission is processed and credits applied, you will receive an email confirmation from Kinetic by Windstream.
Credits to your Kinetic by Windstream account will appear on your next bill.

Lifeline discounts are limited to one per household and vary based on customer location and qualifying service. Proof of participation in a qualifying program or proof of income must be provided. Qualifying programs may include: Supplemental Nutrition Assistance Program (SNAP), Medicaid, Supplemental Security Income (SSI), Federal Public Housing Assistance (FPHA), Veterans Pension or Survivors Pension, Bureau of Indian Affairs (BIA) General Assistance, Tribally-Administered Temporary Assistance for Needy Families (TANF), Tribal Head Start, Food Distribution Program on Indian Reservations. An eligible customer may choose to apply their Lifeline discounts to a qualifying service or carrier. Services include cell phones, mobile Internet, or home Internet (must be at least 20/3 Mbps or at the highest speed available but no lower than 4/1 Mbps). Lifeline credits can only be used with one service and one provider such as for Kinetic Internet by Windstream. Customers must renew/recertify every year to continue in the program. Must be a new Kinetic by Windstream customer to qualify for any additional promotions.
